Ciafil 5mg is a daily oral phosphodiesterase type 5 (PDE5) inhibitor used to manage erectile dysfunction and alleviate lower urinary tract symptoms of benign prostatic hyperplasia.
Tadalafil is a potent, selective, and reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5). During local sexual stimulation, nitric oxide is released, activating guanylate cyclase and increasing cGMP levels in the corpus cavernosum. By blocking PDE5-mediated degradation of cGMP, tadalafil maintains elevated concentrations, promoting smooth muscle relaxation and increased blood inflow to erectile tissues. Similarly, in the bladder neck and prostate gland, PDE5 inhibition decreases smooth muscle tone, enhancing blood perfusion and reducing the obstructive urinary symptoms of benign prostatic hyperplasia.
Purpose
It is indicated for the treatment of erectile dysfunction (ED) in adult men, and for the management of the signs and symptoms of benign prostatic hyperplasia (BPH).

Warnings
**Nitrate Contraindication:** Absolutely contraindicated in patients using any form of organic nitrates or nitric oxide donors (e.g., nitroglycerin, isosorbide dinitrate) due to the risk of severe, life-threatening hypotension. **Cardiovascular Assessment:** Not recommended for men with unstable cardiovascular disease, recent myocardial infarction (within 90 days), or stroke (within 6 months). **Priapism Warning:** Instruct patients to seek immediate medical intervention if an erection persists continuously for more than 4 hours.
Side Effects
**Central Nervous System:** Headache, dizziness, flushing, and transient back pain or myalgia. **Gastrointestinal:** Dyspepsia, acid reflux, and mild abdominal discomfort. **Ophthalmic & Otic:** Nasal congestion, blurred vision, altered color perception (rare), and sudden decrease or loss of hearing.
