Daplozmet XR is a combination oral antidiabetic formulation engineered to provide dual-pathway glycemic control for type 2 diabetes management. The active pharmaceutical ingredients operate complementarily: Dapagliflozin is a highly selective sodium-glucose cotransporter 2 (SGLT2) inhibitor that targets the renal proximal tubules, reducing renal reabsorption of filtered glucose and lowering the renal threshold for glucose, thereby promoting therapeutic glucosuria to remove excess glucose through urine. Concurrently, Metformin Hydrochloride operates as a biguanide in an extended-release matrix to reduce hepatic glucose production (gluconeogenesis), decrease intestinal glucose absorption, and enhance peripheral insulin sensitivity by upregulating glucose uptake and utilization in skeletal muscles.

Purpose

Indicated as an adjunct to diet and exercise to improve glycemic control in adult patients with Type 2 Diabetes Mellitus.

Warnings

Metformin accumulation poses a critical risk of lactic acidosis, a rare but severe metabolic emergency; monitor renal function (eGFR) prior to initiation and periodically thereafter. Strictly contraindicated in patients with type 1 diabetes, diabetic ketoacidosis, severe renal impairment (eGFR less than 30 mL/min/1.73m²), or conditions predisposing to tissue hypoxia. Dapagliflozin increases the risk of euglycemic diabetic ketoacidosis (DKA); discontinue immediately if signs of metabolic acidosis occur, even if blood glucose levels appear normal. Monitor for signs of intravascular volume depletion, hypotension, necrotizing fasciitis of the perineum (Fournier's gangrene), and severe urinary tract infections. Temporarily discontinue before intravascular iodinated contrast imaging procedures or elective major surgeries.

Side Effects

Common: Nausea, diarrhea, flatulence, abdominal discomfort, transient metallic taste, localized headache, increased frequency or volume of urination (polyuria), genital mycotic infections (yeast infections), or mild urinary tract infections. Rare: Lactic acidosis, euglycemic diabetic ketoacidosis, severe volume depletion leading to acute kidney injury, or necrotizing fasciitis of the perineum.

Storage Instructions

Store in a cool, dry place below 30°C (86°F), well protected from moisture, intense humidity, and direct sunlight. Keep the blister pack securely stored inside its original carton shell to preserve formulation stability. Keep out of reach of children.
