Dologab is an analogue gabapentinoid medication used to manage neuropathic pain and control partial onset seizures.
Pregabalin is a structural analogue of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA), though it does not bind directly to GABA receptors or alter GABA uptake. Instead, it binds with high affinity to the alpha-2-delta subunit of voltage-gated calcium channels within central nervous system tissues. By binding to these specific sites, it modulates calcium influx into hyperexcitable presynaptic neurons, subsequently reducing the calcium-dependent release of excitatory neurotransmitters such as glutamate, norepinephrine, and substance P. This stabilizes neuronal cell membranes and diminishes aberrant pain signalling pathways.
Purpose
It is indicated for the management of peripheral and central neuropathic pain (including diabetic neuropathy and postherpetic neuralgia), fibromyalgia, and as adjunctive therapy for adults presenting with partial seizures.

Warnings
**Gradual Discontinuation Required:** Abrupt cessation must be avoided; the dose should be tapered gradually over a minimum of one week to minimize the risk of rebound seizures, insomnia, anxiety, and hyperhidrosis. **CNS Depression:** Patients must be warned that it causes substantial dizziness and somnolence, increasing the risk of accidental falls in elderly populations. **Suicidal Ideation:** Monitor patients closely for emergence or worsening of depression, suicidal thoughts, or unusual changes in mood or behavior.
Side Effects
**Central Nervous System:** Dizziness, somnolence, ataxia, fatigue, tremor, headache, and blurred vision. **Metabolic & Systemic:** Increased appetite, subsequent weight gain, and peripheral edema (swelling of the extremities). **Gastrointestinal:** Dry mouth, constipation, and flatulence.
