Elzanor Txr 10/5/1000mg (14 Tablet / Pack) is an oral triple-combination metabolic therapy containing empagliflozin, linagliptin, and extended-release metformin hydrochloride, formulated for advanced glycemic control. It functions via three complementary mechanisms: empagliflozin inhibits sodium-glucose co-transporter 2 (SGLT2) in the kidneys to reduce renal glucose reabsorption and promote urinary glucose excretion; linagliptin inhibits dipeptidyl peptidase-4 (DPP-4) to elevate active incretin hormones, enhancing insulin secretion and suppressing glucagon output; and metformin reduces hepatic gluconeogenesis while improving peripheral insulin sensitivity.
Purpose
To improve glycemic control in adult patients presenting with type 2 diabetes mellitus when multi-component therapy is indicated.

Warnings
Swallow the tablets whole with a meal to optimize the extended-release profile and minimize gastrointestinal side effects; do not crush or chew. Discontinue therapy immediately and evaluate for ketoacidosis if symptoms like unexplained nausea, vomiting, abdominal pain, or dyspnea occur. Assess baseline renal function (eGFR) prior to initiation and monitor periodically, as the empagliflozin component depends on renal clearance. Educate patients on maintaining optimal perineal hygiene to mitigate the elevated risk of genital mycotic infections.
Side Effects
Gastrointestinal disturbances including mild nausea, abdominal discomfort, flatulence, temporary diarrhea, or an altered metallic taste. Increased risk of genitourinary tracts infections, including vulvovaginal candidiasis, balanitis, or mild urinary tract infections. Mild volume depletion symptoms such as orthostatic lightheadedness, increased urinary frequency, or dry mouth. Rare but severe metabolic complications including lactic acidosis (associated with metformin) or euglycemic diabetic ketoacidosis.
