Elzanor Txr 12.5/2.5/1000mg (14 Tablet / Pack) is an oral triple-combination antihyperglycemic medication containing empagliflozin, linagliptin, and extended-release metformin hydrochloride, structured for personalized maintenance therapy. It combines the osmotic glucosuric effect of an SGLT2 inhibitor (empagliflozin) with the glucose-dependent insulinotropic response of a DPP-4 inhibitor (linagliptin) and the biguanide-mediated reduction of hepatic glucose output (metformin XR). This integrated pharmacodynamic profile optimizes daily plasma glucose levels, reduces HbA1c, and provides cardiovascular benefits without increasing the inherent risk of hypoglycemic episodes.

Purpose

To manage type 2 diabetes mellitus in adults requiring structured triple therapy to maintain target hemoglobin A1c levels.

Warnings

Must be administered with food to optimize absorption and protect the stomach lining from metformin-induced irritation. Avoid excessive alcohol consumption during therapy, as it can significantly potentiate the risk of metformin-associated lactic acidosis. Temporarily withhold this medication prior to and following radiological imaging procedures involving iodinated contrast media. Monitor for signs of volume depletion and dehydration, especially in elderly patients or those concurrently taking loop diuretics.

Side Effects

Metabolic and gastrointestinal side effects including mild nausea, transient diarrhea, abdominal cramping, or minor loss of appetite. Genitourinary adjustments including a higher incidence of fungal infections (thrush) and increased daily urinary volume. Respiratory and systemic symptoms such as a mild nasopharyngitis, upper respiratory irritation, or temporary physical lethargy. Very rare but critical risks including severe arthralgia, acute pancreatitis, euglycemic ketoacidosis, or systemic lactic acidosis.
