A nonsteroidal selective mineralocorticoid receptor antagonist used to slow kidney disease progression and lower cardiovascular risks in type 2 diabetes.
Fenhil contains Finerenone, a nonsteroidal, highly selective antagonist of the mineralocorticoid receptor (MR). It operates by blocking MR overactivation driven by excessive aldosterone and cortisol levels, which occurs in both renal epithelial tissues and nonepithelial cardiorespiratory structures. By preventing MR-mediated gene transcription, it directly dampens downstream pro-inflammatory and pro-fibrotic pathways, successfully mitigating progressive glomerular fibrosis, tubulointerstitial decline, and pathological cardiac remodeling.
Purpose
Indicated to reduce the risk of sustained eGFR decline, end-stage kidney disease (ESKD), cardiovascular death, non-fatal myocardial infarction, and hospitalization for heart failure in adult patients with chronic kidney disease (CKD) associated with type 2 diabetes.

Warnings
Can induce severe, life-threatening hyperkalemia; treatment must not be initiated if baseline serum potassium levels are greater than 5.0 mEq/L. Serum potassium and estimated glomerular filtration rate (eGFR) must be evaluated prior to initiation and monitored closely throughout treatment. Strictly contraindicated in patients with adrenal insufficiency (Addison's disease) or those receiving concurrent strong CYP3A4 inhibitors like ketoconazole or clarithromycin. Avoid co-administration with potassium supplements, potassium-sparing diuretics, or other MRAs. Avoid consumption of grapefruit or grapefruit juice.
Side Effects
Common: Hyperkalemia (elevated serum potassium), hyponatremia (low sodium), hypotension (low blood pressure), hyperuricemia, or transiently decreased glomerular filtration rate (eGFR). Rare: Severe hyperkalemic cardiac arrhythmias or acute declines in hemoglobin concentration.
