Fildil contains Tadalafil, a potent phosphodiesterase type 5 (PDE5) inhibitor. It works by blocking the PDE5 enzyme, which is responsible for the degradation of cyclic guanosine monophosphate (cGMP) in smooth muscle cells. By preventing cGMP breakdown, it enhances the signaling pathway of nitric oxide, leading to prolonged smooth muscle relaxation and increased blood flow to the corpus cavernosum of the penis to enable erection, as well as relaxing the vascular smooth muscle beds of the prostate and bladder neck.

Purpose

Indicated for the treatment of erectile dysfunction (ED), symptoms of benign prostatic hyperplasia (BPH), or concurrent ED and BPH presentations in adult men.

Warnings

Strictly contraindicated in patients concurrently using any form of organic nitrates (such as nitroglycerin or isosorbide dinitrate) or nitric oxide donors, as it can trigger severe, life-threatening drops in blood pressure. Do not co-administer with guanylate cyclase stimulators like riociguat. Use with extreme caution in patients with severe hepatic or renal impairment, recent cardiovascular events, or anatomical penis deformations. Patients must seek immediate emergency medical care if an erection persists for greater than 4 hours (priapism) to prevent permanent tissue damage.

Side Effects

Common: Localized headache, dyspepsia (indigestion), back pain, myalgia (muscle aches), facial flushing, nasal congestion, or pain in the limbs. Rare: Priapism, sudden decrease or loss of vision due to non-arteritic anterior ischemic optic neuropathy (NAION), or sudden hearing loss.
