Gefahil contains Gefapixant, a first-in-class selective antagonist of the P2X3 receptor (and to a lesser degree, the P2X2/3 receptor subtype). P2X3 receptors function as ATP-gated ion channels located primarily on sensory C-fibers of the vagus nerve within the airway mucosal lining. Under conditions of chronic inflammation or mechanical irritation, excess extracellular ATP is released, binding to P2X3 receptors and initiating a damage signal that triggers the cough reflex. Gefapixant blocks this extracellular ATP binding, successfully downregulating hyperactive sensory nerve activation to suppress refractory or unexplained cough loops.

Purpose

Indicated for the treatment of refractory chronic cough (RCC) or unexplained chronic cough (UCC) in adult patients whose symptoms have persisted despite extensive diagnostic management.

Warnings

Taste-related adverse events (including metallic taste, taste distortion, or total loss of taste) are exceptionally common and can be profound; patients should be counseled that these symptoms are typically transient and fully reversible upon drug discontinuation. Structure contains a sulfonamide moiety; use with caution in patients with a history of documented sulfonamide hypersensitivity. Evaluate clinical utility individually if an acute lower respiratory tract infection develops during the course of treatment.

Side Effects

Common: Dysgeusia (taste distortion/metallic taste), ageusia (loss of taste), hypogeusia, upper respiratory tract infections, dizziness, dry mouth, nausea, diarrhea, oropharyngeal pain, or salivary hypersecretion. Rare: Urinary calculus (kidney or bladder stones) or localized cutaneous hypersensitivity reactions.
