Hitica contains Ticagrelor, a cyclopentyltriazolopyrimidine that operates as a reversible, direct-acting P2Y12 adenosine diphosphate (ADP) receptor antagonist. It selectively binds to platelet P2Y12 receptors to prevent extracellular ADP from initiating downstream signal transduction, effectively blocking platelet activation and subsequent macro-thrombus aggregation. Because its binding is completely reversible, it allows platelet function to recover more rapidly upon discontinuation compared to irreversible thienopyridines like clopidogrel.

Purpose

Indicated to reduce the risk of cardiovascular death, myocardial infarction (heart attack), and stroke in patients with acute coronary syndrome (ACS) or a history of myocardial infarction.

Warnings

Significantly amplifies the risk of serious bleeding; strictly contraindicated in patients with active pathological bleeding (such as peptic ulcer disease) or a known history of intracranial hemorrhage. Use with caution in patients with a history of asthma or chronic obstructive pulmonary disease (COPD) due to a known risk of drug-induced dyspnea. Avoid use in patients with severe liver impairment. Ticagrelor can cause bradyarrhythmias; monitor heart rate regularly in patients with sick sinus syndrome or high-grade AV blocks unprotected by a pacemaker. Discontinue therapy at least 5 days prior to elective major surgery.

Side Effects

Common: Dyspnea (shortness of breath), easy bruising, minor epistaxis (nosebleeds), gastrointestinal bleeding, localized headache, dizziness, nausea, or diarrhea. Rare: Severe intracranial hemorrhage, ventricular pauses/bradycardia, acute thrombocytopenia, or hypersensitivity bronchospasm.
