Cephalexin is a semi-synthetic, first-generation cephalosporin antibiotic that provides bactericidal action against a wide array of Gram-positive and select Gram-negative organisms. It works by binding covalently to specific penicillin-binding proteins (PBPs) located within the inner bacterial cell wall. This binding inhibits the final transpeptidation enzyme phase of peptidoglycan synthesis, terminating structural cross-linking. Consequently, the bacterial cell wall weakens, experiences osmotic instability, and undergoes rapid cell lysis during active cellular division.

Purpose

It is indicated for the treatment of respiratory tract infections, otitis media, skin and soft tissue structures, bone and joint infections, and uncomplicated genitourinary tract infections caused by susceptible pathogens.

Warnings

**Cross-Hypersensitivity:** Use with caution in patients with a documented history of penicillin allergy, as cross-reactivity can occur in a small percentage of individuals. **Colitis Warning:** Prolonged use can result in overgrowth of non-susceptible organisms, including Clostridioides difficile-associated diarrhea (CDAD). **Renal Adjustment:** Dosage modifications are required in patients presenting with moderate to severe renal impairment.

Side Effects

**Gastrointestinal:** Diarrhea, loose stools, nausea, vomiting, dyspepsia, and abdominal pain. **Allergic & Dermatological:** Maculopapular skin rash, urticaria, pruritus, and rare erythema multiforme. **Hematological & Hepatic:** Transient eosinophilia, neutropenia, and mild elevations in hepatic transaminases (ALT/AST).
