Valsartan is an orally active, potent, and specific angiotensin II receptor blocker (ARB) that exhibits high affinity for the angiotensin II type 1 ($AT_1$) receptor subunit. By selectively binding to the $AT_1$ receptor in tissues such as vascular smooth muscle and the adrenal glands, it completely antagonizes the physiological actions of angiotensin II. This blockade inhibits vasoconstriction, reduces total peripheral resistance, curtails aldosterone synthesis and secretion, and diminishes renal sodium reabsorption. Its pharmacological activity effectively lowers systemic blood pressure and reduces cardiac afterload independent of angiotensin II synthesis pathways.

Purpose

It is indicated for the treatment of essential hypertension in adults and children, the management of heart failure (NYHA Class II-IV), and to improve survival following a myocardial infarction.

Warnings

**Fetal Toxicity:** Strictly contraindicated during the second and third trimesters of pregnancy as it can cause fetal injury, oligohydramnios, and neonatal renal failure. **Hyperkalemia Risk:** Avoid concomitant use with potassium supplements or potassium-sparing diuretics unless serum potassium levels are strictly monitored. **Renal Assessment:** Renal function and serum creatinine levels should be regularly evaluated, especially in patients with pre-existing renal artery stenosis or severe congestive heart failure.

Side Effects

**Cardiovascular & Renal:** Orthostatic hypotension, dizziness, vertigo, and transient reductions in renal function (elevated BUN/creatinine). **Metabolic:** Hyperkalemia and mild fatigue. **Respiratory & Systemic:** Cough (significantly less frequent than with ACE inhibitors), upper respiratory tract infections, and asthenia.
