Nimoden is a highly specialized cerebrovascular calcium channel blocker formulated to prevent ischemic neurological deficits following intracranial hemorrhages. The active pharmaceutical ingredient, Nimodipine, is a dihydropyridine derivative with a highly lipophilic molecular structure, enabling it to rapidly cross the blood-brain barrier into cerebrospinal fluid. It operates by binding specifically to L-type voltage-gated calcium channels on cerebral vascular smooth muscle cells, inhibiting the transmembrane influx of extracellular calcium ions. This direct blockade promotes arterial smooth muscle relaxation and targeted vasodilation, successfully reversing or mitigating cerebral vasospasm and preserving cerebral blood perfusion to ischemic neuronal tissues.

Purpose

Indicated for the prevention and treatment of ischemic neurological impairments secondary to cerebral vasospasm following aneurysmal subarachnoid hemorrhage (bleeding around the brain).

Warnings

Contraindicated in patients who have suffered a myocardial infarction (heart attack) or an episode of unstable angina within the preceding month. Avoid concurrent administration of strong CYP3A4 inhibitors (such as ketoconazole, ritonavir, or clarithromycin) or strong CYP3A4 inducers (such as rifampicin, phenytoin, or carbamazepine) as they severely alter plasma concentrations. Use with extreme caution in patients with hepatic cirrhosis, cerebral edema, or significantly elevated intracranial pressure. Do not consume grapefruit juice or grapefruits within 4 days of starting or during therapy, as it can cause dangerous increases in systemic bioavailability. Monitor blood pressure closely during initiation.

Side Effects

Hypotension (low blood pressure), facial flushing, localized headache, changes in heart rate (tachycardia or bradycardia), gastrointestinal upset, or transient sweating.

Storage Instructions

Store in a cool, dry place below 30°C (86°F), completely protected from moisture and direct sunlight. Keep the tablets inside their original protective blister film until immediate administration to prevent light-induced decomposition. Keep out of reach of children.
