Onato V Hct 5/160/25mg (28 Tablet / Pack) is a triple-combination antihypertensive oral medication designed for the management of moderate-to-severe essential hypertension. It works through three distinct pharmacological mechanisms: amlodipine blocks calcium influx into vascular smooth muscle to cause vasodilation, valsartan blocks the angiotensin II type 1 receptor to prevent vasoconstriction, and hydrochlorothiazide inhibits renal sodium reabsorption to reduce total intravascular volume. This synergistic triad significantly lowers peripheral vascular resistance and systemic blood pressure.

Purpose

To treat essential hypertension in adult patients whose blood pressure is not adequately controlled on dual-combination therapies.

Warnings

Discontinue the medication immediately upon confirmation of pregnancy due to the risk of fetal toxicity and severe injury caused by ARBs. Can cause severe symptomatic hypotension or dizziness, particularly in volume-depleted, salt-restricted, or elderly individuals. Regularly monitor serum electrolytes, blood urea nitrogen, and creatinine clearance as thiazide diuretics can precipitate electrolyte imbalances. Use with caution in patients with severe renal or hepatic impairment, and avoid concurrent use with potassium supplements without medical advice.

Side Effects

Peripheral edema or localized fluid swelling around the ankles and lower feet due to amlodipine-induced vasodilation. Dizziness, lightheadedness, headache, or orthostatic hypotension upon changing postures from sitting to standing. Electrolyte disturbances including hypokalemia, hyponatremia, hyperuricemia, or transient blood glucose elevations. Gastrointestinal upset such as mild nausea, dyspepsia, diarrhea, or temporary loss of appetite.
