Poze Met is a dual-action oral antihyperglycemic combination targeting insulin resistance across multiple organic pathways. **Pioglitazone** acts as a thiazolidinedione agonist for PPAR-gamma nuclear receptors, which upregulates genes responsible for peripheral glucose disposal, significantly increasing insulin sensitivity within skeletal muscles and adipose tissues. Complementing this, **Metformin Hydrochloride** is a biguanide that primarily works by activating AMP-activated protein kinase (AMPK). This enzymatic activation downregulates hepatic gluconeogenesis and glycogenolysis, decreases intestinal absorption of dietary glucose, and moderately enhances peripheral insulin-mediated glucose uptake.

Purpose

It is indicated as an adjunct to diet and exercise to achieve glycemic control in adult patients with type 2 diabetes whose glucose levels are inadequately managed by metformin or pioglitazone monotherapy.

Warnings

**Lactic Acidosis:** Metformin accumulation can cause rare but life-threatening lactic acidosis; contraindicated in patients with severe renal impairment (eGFR < 30 mL/min/1.73m²), acute heart failure, or severe dehydration. **Fluid Retention:** Contraindicated in patients with active or severe congestive heart failure due to pioglitazone-associated fluid expansion. **Iodinated Contrast:** Discontinue therapy temporarily prior to, or at the time of, intravascular iodinated contrast imaging procedures.

Side Effects

**Gastrointestinal:** Diarrhea, nausea, vomiting, abdominal bloating, flatulence, a metallic taste, and anorexia. **Metabolic & Fluid:** Weight gain, localized peripheral edema, and long-term Vitamin B12 deficiency due to malabsorption. **Musculoskeletal:** Myalgia and an elevated risk of distal upper/lower limb bone fractures in women.
