Riam 400mg (100 Tablet / Pack) is an oral nitroimidazole antimicrobial medication containing metronidazole, utilized for treating targeted anaerobic bacterial and protozoal infections. It works through passive cellular diffusion into the invading microorganism, where its nitro group is enzymatically reduced into reactive cytotoxic intermediates that directly bind to microbial DNA. This destructive chemical interaction disrupts the helical DNA structure, inhibits nucleic acid synthesis, and precipitates rapid cell death in susceptible pathogens.

Purpose

To treat acute and chronic amoebiasis, giardiasis, trichomoniasis, anaerobic systemic infections, and bacterial vaginosis.

Warnings

Strictly avoid all alcohol consumption and alcohol-containing medications during therapy and for at least 48 hours afterward to prevent a severe disulfiram-like reaction. Use with extreme caution in patients with active central nervous system disorders or epilepsy due to the risk of dose-dependent neurotoxicity. Complete the entire prescribed course of treatment even if symptoms resolve early, helping prevent the development of drug-resistant microbial strains. Monitor prothrombin time and adjust dosages carefully in patients receiving concurrent oral anticoagulant therapy like warfarin.

Side Effects

Gastrointestinal symptoms including a persistent unpleasant metallic taste, nausea, vomiting, anorexia, or epigastric distress. Central nervous system effects such as transient dizziness, lightheadedness, headache, vertigo, or mild coordination changes. Harmless darkening or deep reddish-brown coloration of the urine caused by water-soluble metabolites of metronidazole. Dermatological or systemic signs including mild skin rashes, pruritus, temporary leukopenia, or a feeling of physical fatigue.
