Sitaformin 50/500mg (14 Tablet / Pack) is an oral fixed-dose combination antihyperglycemic medication containing sitagliptin phosphate and metformin hydrochloride, formulated for type 2 diabetes management. It operates through two distinct, synergistic mechanisms: sitagliptin acts as an oral dipeptidyl peptidase-4 (DPP-4) inhibitor, preventing the degradation of incretin hormones (GLP-1 and GIP) to enhance glucose-dependent insulin secretion and lower glucagon levels; concurrently, metformin reduces hepatic glucose production, decreases intestinal glucose absorption, and enhances peripheral insulin sensitivity by upgrading tissue glucose uptake.

Purpose

To improve glycemic control in adult patients with type 2 diabetes mellitus as an adjunct to diet and exercise.

Warnings

Take with meals to minimize gastrointestinal discomfort and reduce the incidence of abdominal side effects associated with metformin. Discontinue use immediately if severe, persistent abdominal pain radiating to the back occurs, which may indicate acute pancreatitis. Assess renal function (eGFR) prior to initiation and avoid use if renal clearance falls below standard safety parameters. Avoid excessive alcohol intake while on this medication due to the elevated risk of inducing metabolic lactic acidosis.

Side Effects

Gastrointestinal side effects including nausea, vomiting, abdominal bloating, temporary diarrhea, or a transient metallic taste. Metabolic adjustments such as hypoglycemia when used in combination with insulin or sulfonylureas, and low vitamin B12 levels. Respiratory or systemic symptoms including mild nasopharyngitis, upper respiratory tract irritation, or a tension headache. Exceptional systemic complications such as severe hypersensitivity reactions, acute pancreatitis, or metformin-induced lactic acidosis.
