Stelomin 50/500mg (10 Tablet / Pack) is an oral fixed-dose combination antidiabetic medication containing sitagliptin and metformin hydrochloride, formulated to optimize glycemic control. It works synergistically: sitagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that prolongs the action of incretin hormones, thereby increasing glucose-dependent insulin release and suppressing glucagon secretion. Concurrently, metformin acts as a biguanide that decreases hepatic glucose output, reduces intestinal glucose absorption, and improves target cell insulin sensitivity, addressing multiple pathophysiological defects in type 2 diabetes.

Purpose

To improve glycemic control in adult patients with type 2 diabetes mellitus when diet, exercise, and single-agent therapy are insufficient.

Warnings

Administer strictly with meals to minimize metformin-associated gastrointestinal distress and ensure consistent absorption profiles. Renal function (eGFR) must be assessed prior to initiation and monitored regularly, as metformin accumulates in cases of renal impairment. Temporarily suspend therapy prior to radiological studies utilizing intravascular iodinated contrast materials to prevent acute renal failure. Educate patients to immediately recognize and report symptoms of lactic acidosis, such as profound muscle weakness, hypothermia, or dyspnea.

Side Effects

Frequent gastrointestinal symptoms including transient nausea, diarrhea, abdominal bloating, indigestion, or a metallic taste in the mouth. Potential metabolic adjustments such as minor weight fluctuations or an increased risk of mild hypoglycemia when combined with other agents. Respiratory and systemic issues including a slightly higher incidence of upper respiratory tract infections or nasopharyngitis. Very rare but critical risks including severe acute pancreatitis, debilitating joint pain, or life-threatening metformin-induced lactic acidosis.
