Stelomin Plus 50/100mg (10 Tablet / Pack) is an advanced oral fixed-dose combination antihyperglycemic agent combining sitagliptin and a high dose of metformin hydrochloride for intensive metabolic regulation. It operates through complementary pathways: sitagliptin provides incretin-mediated, glucose-dependent enhancement of insulin secretion via DPP-4 inhibition, preventing dangerous postprandial glucose spikes. Meanwhile, the metformin component vigorously suppresses hepatic gluconeogenesis and improves peripheral glucose uptake in muscle tissues, breaking the cycle of severe insulin resistance often seen in advanced type 2 diabetes.

Purpose

To provide intensive glycemic control in adult patients with type 2 diabetes who require concurrent intervention with a DPP-4 inhibitor and biguanide.

Warnings

Must be taken exactly as prescribed alongside food to heavily buffer the stomach against the potent gastrointestinal effects of high-dose metformin. Discontinue use immediately and seek emergency medical evaluation if severe, radiating abdominal pain occurs, indicating potential pancreatitis. Strictly avoid excessive acute or chronic alcohol consumption, which drastically elevates the risk of severe metabolic lactic acidosis. Monitor serum vitamin B12 levels periodically during long-term therapy, as continuous high-dose metformin can interfere with its absorption.

Side Effects

Gastrointestinal disturbances including prominent nausea, acute diarrhea, stomach cramping, flatulence, or temporary loss of appetite. Neurological and systemic symptoms such as mild tension headaches, physical asthenia, or upper respiratory tract irritation. Increased susceptibility to localized infections such as minor pharyngitis or localized urinary tract infections. Rare but severe systemic adverse events including profound hypersensitivity reactions, lactic acidosis, or severe arthralgia.
