Valmera 10mg (30 Tablet / Pack) is an oral voltage-gated calcium channel modulator containing mirogabalin, indicated for the targeted management of chronic neuropathic pain. It works by binding with high affinity and selectivity to the alpha-2-delta-1 ($\alpha_2\delta_1$) subunit of voltage-gated calcium channels located on presynaptic neurons in the central nervous system. This targeted binding reduces influx of calcium into presynaptic terminals, suppresses excessive release of excitatory neurotransmitters (such as glutamate and substance P), and attenuates hyperexcitable pain signaling pathways.

Purpose

To manage peripheral neuropathic pain, including diabetic peripheral neuropathy, postherpetic neuralgia, and central neuropathic pain.

Warnings

Can cause significant somnolence, dizziness, and cognitive blurring; warn patients against driving or operating machinery until individual tolerance is established. Avoid sudden, abrupt cessation of therapy; taper the dose gradually over at least 1 week to minimize the risk of withdrawal symptoms. Reduce standard starting dosages and perform careful maintenance monitoring in patients presenting with moderate-to-severe renal impairment. Avoid concurrent use of central nervous system depressants, opioids, or alcohol, as they can severely potentiate sedative effects.

Side Effects

Central nervous system reactions including prominent drowsiness, postural dizziness, tension headaches, or general physical fatigue. Gastrointestinal disruptions such as mild nausea, a feeling of dry mouth, constipation, or minor abdominal discomfort. Metabolic or peripheral changes including localized peripheral edema, fluid retention, weight gain, or transient blurred vision. Neurological or psychiatric variations including unsteadiness, gait disturbances, minor sleep changes, or transient memory clouding.
